General Context

MSF-OCBA intervened firstly in Western Equatoria State (WES) with a vertical Human African Trypanosomiasis (HAT) project from the end of 2004 in Tambura, when the sero-prevalence was over 2%, considered the epidemic threshold. After two years of activities and the result of a reduced prevalence (<0 2006.="" another="" december="" handed="" hat="" in="" ing0="" nbsp="" over="" program="" span="" the="" to="" was="">
Since October 2006 a HAT inpatient program started in Yambio County Hospital (YCH). Between June and December 2008, a project to support the PHCCs of Sakure and Gangura was implemented, justified by the arrival of refugees and IDPs fleeing from LRA attacks in DRC and South Sudan. In 2008, it was decided to close the HAT project in Yambio due to low prevalence rates in the area. At the same time it was also decided to stop the activities in Sakure and Gangura.

Current Activities

MSF-OCBA re-started new activities in Yambio (Western Equatoria State) during the month of March 2009 to respond to the humanitarian consequences of the activities of the LRA in WES. A Memorandum of Understanding signed with the Ministry of Health of the Government of South Sudan dated the 6th March 2009 legitimates our action.

The overall objective of MSF-OCBA activities in WES is now to assist the most vulnerable population, namely children under 5 years old and pregnant women. MSF is supporting the MoH in the State hospital in Maternity and Paediatric wards: General OPD services for children <5years 34="" 3="" a="" activities="" addition="" aligned="" and="" available="" behaviour="" care="" centres="" community="" education="" families="" for="" fp="" general="" health="" hiv="" implement="" improve="" in="" mnch="" msf="" network="" o:p="" of="" outreach="" patients="" phccs="" pmtct.="" pnc="" primary="" promotion="" public="" quality="" seeking="" services.="" sgbv="" sti="" strong="" supports="" the="" to="" utilization="" who="" with="" workers="">

Emergency response and medical and humanitarian assistance to populations affected by violence and/or natural disasters. Assistance defined as the provision of the minimal package of essential primary and secondary health care. The response capacity will be accompanied by constant lobbying of other humanitarian actors pushing them to assume their responsibilities and coordinate their actions. Witnessing and advocacy will be a key complement of the above-mentioned strategies in order to improve the situation of the population.

Main purpose
As Clinical officer or Nurse with anaesthesia skills he/she will be responsible for the anaesthesia activities in the OT of YSH according to MSF guidelines and protocols in cooperation with the MoH.
The main focus is to provide safe anaesthesia to patients undergoing surgery and to train MoH national staff appointed to the anaesthesia department.
He/she will work in close cooperation with the MSF MD/surgeon and the MoH surgeons to provide the best care in the operation theatre.
He/She will work under on call system.
This will be done in accordance to the context, MSF protocols, procedures and the universal hygiene standards.


Accountabilities
General:
Ensure the implementation and standardization of MSF clinical policies, tools, and hygiene protocols in order to minimize clinical risks and to granting quality of patient case management in the OT
Promote and maintain confidentiality regarding all patient cases and records and consultation
Participate in the planning and supervising, in close coordination with other supervisors, the HR processes (sizing, staff shifts, recruitment, training, evaluation, development and communication) of the team in order to ensure both the sizing and the amount of knowledge required, improve people triage capabilities, their motivation and commitment and their active participation in teaching the targeted population on therapeutic and preventive protocols.
Training of national staff in the department
            Ensure training of the national staff in the department following the established curriculum
Provide qualitative theoretical and practical lessons for the staff
Fill in attendance protocol to ensure the staff receives the training
Supervise anaesthesia given by the national staff during duty
Evaluate the skills regular
Perform theoretical and practical test for the national staff
Coordinate with the MoH surgeons and MSF surgeon the possible procedures given by national staff
Ensure that all partners are aware of the training and respect the effort and time

Pre-surgical consultation
Respects medical confidentiality and provides the patients with information on their illnesses and  treatments
Carries out patients’ pre-surgical consultations including filling the records, request laboratory investigation if necessary
Evaluates the risks of operating in collaboration with the surgeon
Decides on the type of anaesthetic to be used

 Anaesthesia
Ensure all material is available to conduct the anaesthesia/ react to emergency
Prepares the drugs, material and equipment required
Administers anaesthetic drugs according to the techniques chosen beforehand and following the rules of hygiene and asepsis in force
Monitors patients throughout the interventions

Immediate post-surgical supervision
Ensure post-surgery surveillance of the patient
Hand over all information to the nurse in charge of recovery
Evaluates patients’ pain and sets up a protocol of post-surgical pain relief
Draws up post-surgical prescriptions according to his specialization (resuscitation, ATB, pain relief…)

Post-surgical care
Does  rounds with the surgeon once or twice a day
Discusses therapeutic orientations with the department manager
Renews prescriptions, checks they are followed
Pays particular attention to the proper management of pain, systematically evaluating pain once or several times a day and adapting prescriptions to the levels of pain

Intensive care
Provides post-surgical follow up of surgical patients in the facility (rounds, prescriptions, etc.)

Hygiene
Checks that rules of hygiene for the protection of patients and staff are followed
Knows accidental blood exposure policy and procedure, and implements when necessary.

Pharmaceutical material
Sets up and monitors the stock of anaesthetic material
Supervises the hygiene and integrity of anaesthetic material: decontamination, sterilization, storage conditions, etc.
Checking that the anaesthesia cabinet remains cleaned and re-stocked between each patient
Draws up pharmacy orders according to needs
Supervises the follow up and traceability of sedatives (register, empty phials)
Participates in formulation of medical order for anaesthesia meds and supplies if activity manager.

Recording & reporting
In collaboration with the surgeon, carries out quantitative surgical-anaesthesia data collection
Draws up and sends out reports for the monthly follow up of peri-surgical mortality
Draws up and sends out monthly reports on the quality of anaesthesia/pain management
Informs immediately the direct supervisor in the event of medical error
Promotes and maintains confidentiality regarding all patient cases and records
Give monthly report to the MTL on the training progress.

Supports to other departments
Intervenes in support of other services whenever needed  and works particularly in close collaboration with the accident and emergency department
Supports the midwifery & obstetrics teams in maternity department, particularly to resuscitate new-borns
Supervises the management of emergency trolleys
Possibly supports or gives trainings on blood transfusion control, first aid, etc.

Pain management
Is the reference person for pain problems within the health facility

Emergencies
Participates in emergency activity or exploratory visit in or out of the MSF area of work upon request

Others
Participating in personnel meetings



Requirements

Education
Essential clinical officer diploma with additional training in anaesthesia
Experience
Desirable previous clinical supervision experience of at least one year, Essential, 2 years of relevant working experience in the anaesthesia department
Languages             
English, local language is an asset
Knowledge
Desirable computer literacy
Competences
Cross cultural awareness and flexibility / Teamwork / Quality and results orientation / Identification with MSF / Communication skills.
